Abstract

Background: Healthcare is facing a future scenario with an increasing number of elderly people with one or more medical conditions, which places demands on efficiency and safety in the care chain. In society, a transition to a more person-centred care, where the individual is put in focus, has been underway for a decade. The nurse in home care meets the patient at home, which is the most natural environment for person-centred care. In home care, there are several challenges, such as geographical distance to both patient and care staff and a fragmented communication flow, which raises the question of how nurses experience person centered care in municipal home care Purpose: The purpose was to shed light on the nurse's experience of person-centred care in municipal home care Method: A literature review, based on ten qualitative articles selected from nursing-oriented databases, was conducted. HKR's review template was used in the review and the analysis has been carried out in accordance with Friberg's five-step model. Results: The nurse's experiences of person-centred care in municipal home care are reproduced in three main categories and seven subcategories. The meeting with the patient: relationships characterized by friendship, respect and recognition, the importance of continuity. The significance of context: collaboration and communication, the importance of life experience, the importance of self-knowledge. Conditions that have an influence: leadership, knowledge and education. Discussion: The method is discussed according to Shenton's four concepts of credibility. Three findings from the results are discussed based on McCormack and McCance's theoretical framework for person-centred care. The findings are: the importance of continuity, collaboration and communication as well as leadership.
